Distributor/Retailer Application Process
Mika International, Inc. manufactures many of the products stained glass artists use all across the world. You can be assured that Mika stands behind all of our products 100%.
To begin the process we need documentation demonstrating your status as a commercial business to open your account. Below is a list to aid in collecting the pertinent information, along with a
credit application to download, fill out, and return.

Upon successful review of your information you will receive a Mika International price guide which lists terms, and conditions. In addition we will begin sending you information on any current specials, including promotional materials. Mika will also begin to direct any sales inquires to you from customers in your area.
INCLUDE ONE COPY OF EACH OF THE FOLLOWING:
- Current resale tax number
(may substitute a copy of the business license for Canada and states not requiring a resale tax number)
- Federal ID number
(or social security number)
- Voided business check
- Business Card
- Business Letterhead
INCLUDE AT LEAST ONE OR MORE OF EACH OF THE FOLLOWING:
- Store front photo
- Utility Invoice
- Company Brochure
- Yellow Page Advertisement
- Certificate of business liability insurance
